DO I HAVE PREDIABETES?

To see if you are at risk for prediabetes, take the Centers for Disease Control and Prevention (CDC) / American Diabetes Association (ADA) prediabetes risk test.

IF YOU’RE AT RISK

Talk to your primary care provider (PCP) about what to do next. You may be eligible to participate in a Diabetes Prevention Program (DPP). DPP is a scientifically proven program designed to help people with prediabetes lose weight gradually through dietary changes and increased physical activity.

ABOUT YOUR EUTF DPP PLAN BENEFITS

Eligible HMSA members can participate in the Diabetes Prevention Program (DPP) at no cost. To find out if you’re eligible to enroll in the program, talk to your PCP or call HMSA at 948-6499 on Oahu or 1-800-776-4672 toll-free on the Neighbor Islands. Representatives are available Monday thru Friday, 7am – 7pm, and on Saturday, 9am – 1pm.

Kaiser Permanente members who have prediabetes may be eligible to participate in a facility-based or digital-based Diabetes Prevention Program (DPP) without a member fee. Kaiser members may self-refer by calling 432-2260 and ask to speak with a Coach to learn of the various prediabetes programs available including DPP.
